Purpose of Counseling: (Leader states the reason for the counseling, e.g. Performance/Professional or Event-Oriented counseling and includes the leaders facts
and observations prior to the counseling)

Event-Oriented: Violation of article 108 (Loss, damage, destruction or wrongful disposition of military property) of the Uniform Code Of Military
Justice in that on or about (date) at (location) you damaged/lost___________(what happened? Describe in detail).

Key Points of Discussion:


(Damaging, Losing, selling) military property is a crime and it is unacceptable behavior that will not be tolerated. If found
guilty of this offense, the maximum punishment is ______________________. Your misconduct and neglect resulted in
needless expense and has jeopardised the effectiveness of our organization to successfully accomplish the missions at
hand. (I am recommending that the commander impose nonjudicial punishment for your misconduct) OR (You will receive
corrective training as outlined in the plan of action).





You are here by notified that if this type of conduct continues, actions may be initiated under the Uniform Code of Military
Justice to separate you from the Army prior to your scheduled ETS Date IAW AR 635-200.  If you are involuntary
separated, you could receive an Honorable Discharge, a General (Under Honorable Conditions) Discharge, or a Under
Other Than Honorable Conditions Discharge. If you receive a discharge Under Other Than Honorable Conditions, you will
be ineligible for reenlistment and for most benefits, including payments of accrued leave, transitional benefits, the
Montgomery GI Bill, VA benefits, and You may also face difficulty in obtaining civilian employment.

This form will be destroyed upon reassignment (other than rehabilitative transfers), separations at ETS, or upon retirement. For separation requirements and

armed forces who,  without proper authority; (1) sells or otherwise disposes of: (2) willfully or through neglect
damages, destroys, sells, or loses military property;  may be punished under article 108.

Losing, Damaging or Selling Military property is unacceptable and should not be taken lightly. This Army counseling is intended to serve
as a guide to counsel the Soldier that loses a piece of their TA 50 such as knee pads, canteens, ruck sack, and other items of the TA 50.
After you counsel your Soldier you should contact the unit supply Sergeant to write up a statement of charges and get the missing items
replaced immediately if possible you can have the Soldier purchase the missing item at your local Military Clothing Sales Store MCSS and
avoid the statement of charges, but counsel the Soldier either way. If your Soldier is missing a sensitive item like the Chemical Protective
Mask then you should contact your Chain of Command immediately as this must be reported to higher headquarters and likely start an
investigation, the same goes for a missing weapon as the installation will be locked down to find the missing weapon hope this helps
